This is a fully remote position, open to applicants in Texas.
• Design, implement, and oversee comprehensive care management initiatives for long-term care members.
• Create or assist in developing tailored service care plans/service proposals.
• Educate members and their families/caregivers about available services, benefits, procedures, provider guidelines, service options, referrals, and healthcare entitlements.
• Assess member requirements and available resources, recommending or facilitating optimal outcomes.
• Formulate ongoing long-term care strategies while identifying providers, specialists, and community resources.
• Serve as a liaison between members/families/caregivers and care provider teams to ensure service accessibility.
• Offer resource support in areas such as employment, housing, participant direction, independent living, justice, and foster care needs.
• Oversee care plans, monitor member status and outcomes, and suggest updates based on member requirements.
• Engage with long-term care providers and partners to address member needs effectively.
• Gather, record, and maintain member information and care management activities to ensure regulatory compliance.
• Conduct on-site assessments as necessary to evaluate member needs and collaborate with providers/resources.
• Provide constructive feedback to leadership on enhancing care quality and cost-effective service delivery.
• Perform additional duties as assigned.
• Must be a resident of the state of Texas.
• Bachelor's degree and one year of related experience, or equivalent experience gained through applicable knowledge, duties, scope, and skills.
• Direct experience working with individuals with disabilities and/or vulnerable populations with chronic or complex conditions, including children and young adults, within three of the last five years.
