LPN, Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct phone consultations with patients who are part of care management and/or remote patient monitoring programs.

• Offer support and education regarding patients' chronic health conditions.

• Inform patients about their health issues and empower them with lifestyle and behavioral strategies.

• Help patients establish and achieve goals that align with provider-approved care plans.

• Keep accurate and current patient records, documenting interactions and care plans as per protocol.

• Address patient concerns and barriers to care, identifying practical solutions to enhance adherence and outcomes.

• Communicate with patients in a clear, compassionate, and effective manner.

• Adhere to approved workflows for conveying patient needs to healthcare providers.

• Engage in training sessions, team meetings, and quality improvement initiatives.

• Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, following approved partner workflows.

• Manage a designated group of patients throughout the month and assist with additional patients or tasks as assigned.

⛳️ Requirements

• Must hold a current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN).

• Additional state licenses may be necessary.

• A minimum of 3 years of experience in primary care, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring is required.

• Strong communication and interpersonal skills are essential.

• Excellent organizational and time management skills are necessary.

• Proficient in using electronic health records (EHR) and care management software.

• Capable of working independently as well as collaboratively within a team.

• Empathetic with a patient-centered approach to care.

• Reliable internet connection is required.

• Must have a HIPAA-compliant work area.

• Proficient with virtual communication platforms such as Zoom and Slack.

• Availability to work Monday through Friday, from 9:00 am to 5:30 p.m. Eastern Time, unless specified otherwise.
