This is a fully remote position, open to applicants in Virginia.
• Offer consultation-driven behavioral health services within a comprehensive virtual primary care team.
• Perform same-day consultation visits, scheduled video consultations, and targeted interventions for referred patients.
• Provide behavioral intervention suggestions to referring primary care providers.
• Conduct risk evaluations for crisis situations.
• Implement evidence-based interventions for mental health symptoms, behavioral health challenges, chronic disease management, and overall health promotion.
• Engage in collaborative, stepped, and measurement-based care with suitable follow-up.
• Collaborate on clinical pathways and psychoeducational programs for priority populations.
• Create patient education resources for prevalent behavioral health conditions and issues.
• Offer collaborative decision support and interdisciplinary consultation to primary care teams.
• Educate primary care staff on brief behavioral and cognitive behavioral interventions.
• Promote medication adherence and effectiveness.
• Assist with triage, care coordination, and access to external resources.
• Participate in clinical training, quality improvement initiatives, and staff meetings.
• Maintain clinical documentation and necessary administrative paperwork.
• Ensure virtual visibility and availability to the primary care team during clinic hours.
• Provide curbside provider-to-provider consultation via chat or phone.
• Deliver all patient care through HIPAA-compliant video telehealth.
• Master’s degree in Social Work from a CSWE accredited social work program or a Master’s degree in Counseling from a CACREP or CORE accredited counseling program is required.
• Licensed as an LCSW or LPC in the Commonwealth of Virginia, or possess an active out-of-state license with Virginia licensure obtained within 18 months of hire.
• BLS Certification required within 90 days of hire.
• Experience delivering clinical interventions for a diverse array of behavioral health conditions.
• Familiarity with medically-related issues.
• Experience working with pediatric populations and families in pediatric settings.
• Interest in a dynamic primary care environment and interdisciplinary collaboration.
• Proficient in using personal computers and computerized scheduling systems.
• Willingness to maintain flexibility in the clinic schedule.
• Strong interpersonal skills with patients and the primary care team.
• A learning and growth-oriented mindset towards new topics and challenges.
• Enthusiasm for working in primary care as part of a team.
• Must reside in Virginia and provide all care from within the Commonwealth.
• Reliable high-speed internet is necessary for video-based patient care.
• A private, confidential workspace suitable for clinical telehealth is required.
• The schedule consists of 32 patient-facing hours and 8 administrative hours weekly.
• Weekend and/or evening coverage is required as part of the rotation.
• Computer equipment will be provided.
• Base salary plus a wRVU-based incentive model during the first two years of employment.
• BLS Certification required within 90 days of hire.
