Licensed Practical Nurse – Admission / Intake Coordinator

This is a fully remote position, open to applicants in Illinois, +3 more states.

📋 Description

• Manage and evaluate patient referrals for home health, hospice, and infusion services.

• Assess the suitability, eligibility, completeness, and necessary documentation for referrals.

• Facilitate seamless transitions from hospital care to home care services.

• Relay patient requirements and physicians' directives to clinicians to formulate care plans.

• Educate case managers, physicians, hospital personnel, and nursing home staff on qualifying criteria and service accessibility.

• Evaluate patient care needs by utilizing hospital records and referral data.

• Identify educational requirements for patients and their significant others.

• Comply with payer skilled-service and homebound regulations.

• Ensure timely communication of pending referral details to nursing, clinical, rehabilitation, and other teams.

• Coordinate the initiation of care and obtain appropriate multidisciplinary orders.

• Reevaluate patients and documentation to track progress, trends, and necessary interventions.

• Maintain communication with supervisors regarding managed cases.

• Assess environmental risks and respond in accordance with policy.

• Execute other assigned responsibilities.

⛳️ Requirements

• LPN licensure in Missouri, Illinois, Wisconsin, or Oklahoma.

• Graduate from an accredited nursing program or equivalent educational background for licensing.

• Minimum of one year of experience as a licensed practical nurse.

• Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association.

• Strong clinical judgment skills.

• Capability to evaluate referral information for eligibility, appropriateness, and necessary documentation.

• Preferred experience in intake, referral coordination, home health, or care coordination.

• Familiarity with the Epic system is preferred.

• Excellent organizational, prioritization, communication, and attention to detail skills.

• Ability to work independently in a remote setting while collaborating with multidisciplinary teams.

• Flexibility to provide PRN coverage, including availability on weekends and holidays.

• Capacity to meet physical demands such as standing, walking, lifting, patient movement, driving, and keyboard/data entry.
