Licensed Independent Behavioral Health Clinician – Case Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Perform thorough evaluations of members' behavioral health, physical health, and psychosocial requirements.

• Create tailored care plans that enhance health outcomes and promote overall wellness.

• Work in partnership with providers, care teams, and community resources to facilitate appropriate care coordination.

• Utilize clinical judgment to identify and tackle risk factors, obstacles to care, and intricate health issues.

• Employ Motivational Interviewing methods to engage members and foster long-lasting behavioral changes.

• Offer crisis intervention and ensure prompt access to suitable treatment and support services.

• Coordinate behavioral health and substance use disorder services throughout the entire care continuum.

• Use technology and clinical systems to document care and manage member interactions effectively.

⛳️ Requirements

• Master's Degree in Social Work, Counseling, or a related Behavioral Health discipline.

• Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent).

• Over 3 years of direct clinical experience post-master's in hospital, ambulatory, outpatient, or similar behavioral health environments.

• More than 3 years of experience in treating mental health and substance use disorders.

• At least 3 years of experience in crisis intervention and applying Motivational Interviewing techniques.

• Strong technical skills and the ability to document while engaging members over the phone.

• Experience in Managed Care or Utilization Review is preferred.

• Case Management experience is preferred.

• Discharge Planning experience is preferred.

• Experience with complex populations and multidisciplinary teams is preferred.
