Licensed Independent Behavioral Health Clinician, Case Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct thorough evaluations of members' behavioral health, physical health, and psychosocial requirements.

• Create tailored care plans aimed at enhancing health outcomes and overall wellness.

• Collaborate with healthcare providers, care teams, and community resources to ensure effective care coordination.

• Employ clinical judgment to pinpoint and address risk factors, obstacles to care, and intricate health needs.

• Utilize Motivational Interviewing strategies to engage members and promote sustainable behavior change.

• Offer crisis intervention and guarantee prompt access to suitable treatment and support services.

• Coordinate behavioral health and substance use disorder services throughout the care continuum.

• Use technology and clinical systems to document care and oversee member interactions.

⛳️ Requirements

• Master's Degree in Social Work, Counseling, or a related field within Behavioral Health.

• Current, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent).

• Over 3 years of direct clinical experience post-master's in hospital, ambulatory, outpatient, or comparable behavioral health environments.

• More than 3 years of experience in treating mental health and substance use disorders.

• At least 3 years of experience in crisis intervention and applying Motivational Interviewing techniques.

• Strong technical skills with the capability to document while engaging with members over the phone.

• Experience in Managed Care or Utilization Review is preferred.

• Previous Case Management experience is preferred.

• Familiarity with Discharge Planning is preferred.

• Experience working with complex populations and multidisciplinary teams is preferred.
