This is a fully remote position, open to applicants in California.

📋 Description

• Oversee a designated remote patient monitoring and/or chronic care management patient group.

• Ensure precise and timely documentation for every patient interaction in alignment with Welby Health protocols and relevant billing requirements.

• Engage in proactive outreach, review device data, update care plans, and facilitate care coordination.

• Exemplify standards in clinical engagement, documentation, and conduct.

• Supervise the performance, productivity, documentation accuracy, and compliance of the assigned team.

• Track individual and team KPIs, which include clinical engagement hours, unbilled time rates, and compliance in remote patient monitoring.

• Recognize and report any issues in collaboration between Care Managers and Medical Assistants.

• Conduct regular coaching sessions based on performance metrics.

• Manage the distribution of caseloads and address capacity constraints or clinical risk patterns as they arise.

• Oversee onboarding and provide continuous training and coaching.

• Offer patient care coverage during periods of staffing shortages.

• Monitor team productivity, patient enrollment rates, and patient churn.

• Identify and escalate any patient care bottlenecks and potential risks.

• Convert leadership directives into actionable team priorities.

• Provide frontline clinical insights to inform strategy and protocol evaluations.

• Ensure compliance with regulatory standards, payer guidelines, and Welby Health clinical protocols.

• Organize chart audits and ensure feedback loops are closed.

• Maintain knowledge of billing compliance and enforce documentation standards.

• Act as the primary escalation point for concerns regarding clinical quality.

• Foster relationships with clinic administrators, medical directors, and care coordination personnel.

• Lead performance reviews, care coordination discussions, and escalation calls with clients.

• Convert client feedback into operational initiatives.

• Develop client success strategies and execution plans at the team level.

• Deliver comprehensive performance reports.

• Formulate corrective action plans.

• Assist in expansions, renewals, and onboarding of new practices.


⛳️ Requirements

• Bachelor's degree in nursing or a related healthcare field.

• Active and valid registered nurse (RN) license in good standing.

• A minimum of three (3) to five (5) years of clinical experience, with a proven background in care management, chronic disease management, or a population health program.

• Previous experience in a leadership or supervisory clinical role with direct reports, including responsibility for team performance while managing an individual caseload.

• Practical knowledge of remote patient monitoring, chronic care management, principal care management, or similar chronic care program structures and documentation needs.

• Proven ability to drive accountability within cross-functional teams without having direct authority over all contributors.

• Proven capacity to establish and maintain professional relationships with clients or providers.

• Ability to balance personal productivity standards with team and client responsibilities simultaneously.

• Excellent written and verbal communication skills, capable of representing Welby Health credibly in client-facing situations.

• Strong data fluency, with the ability to analyze dashboards, recognize trends, and convey findings to diverse audiences.

• Familiarity with remote physiological monitoring, EMR/EHR systems, and patient engagement tools.

• Experience working with Federally Qualified Health Centers or organizations focused on value-based care.

• Proficient in both verbal and written Spanish.
