This is a fully remote position, open to applicants in Canada.

📋 Description

• Offer assistance to patients and healthcare professionals regarding Patient Support services, specific conditions, and treatment options.

• Aid patients and caregivers with evaluations connected to Patient Support services.

• Serve as a liaison among patients, families, and healthcare providers concerning medication accessibility.

• Respond to inquiries related to program services, disease conditions, and products.

• Deliver continuous support to enhance patient adherence.

• Refer patients to healthcare providers for clinical inquiries that extend beyond program materials.

• Record patient information and interactions in electronic patient records.

• Review and refresh files throughout the patient experience.

• Adhere to case management protocols, which include data gathering, follow-up, monitoring therapy disruptions/discontinuations, adverse event reporting, and liaising with primary healthcare providers.

• Collaborate with reimbursement specialists and physicians to secure funding for therapies and explore alternatives.

• Organize and monitor patient services based on KPIs to accelerate therapy access.

• Engage in specialized therapy training.

• Attend pharmaceutical and patient meetings, conferences, and collaborate with patient associations and advocacy groups.

• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics regarding treatment logistics and issue resolution.

• Communicate with management, team members, and clients regarding patient case management.

• Assist in training new Nurse Case Managers and provide feedback on protocols, databases, and procedures.

• Report adverse events and significant adverse events in accordance with program protocols.

• Carry out other designated duties and responsibilities, which may include standby/on-call work.

⛳️ Requirements

• Diploma in healthcare or a related field; a bachelor’s degree is preferred.

• Active registration as a Licensed Nurse (RN or RPN/LPN) with the College of Nurses in the province of employment is essential.

• Capability to resolve issues and work both independently and collaboratively within a team.

• Strong analytical and problem-solving abilities.

• Proficient in working effectively across various functions and within a compact team.

• Excellent teamwork capabilities and a readiness to provide constructive feedback.
