This is a fully remote position, open to applicants in Washington.
• Conduct health evaluations within the homes of members.
• Interact with individuals on a personal level to help address gaps in care.
• Assess medications and review medical history.
• Execute physical assessments.
• Carry out additional diagnostic tests as requested by the health plan, such as diabetic eye exams and spirometry tests.
• Utilize an iPad along with clinical workflows for each evaluation.
• Facilitate virtual visits when necessary.
• Offer Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screenings.
• Travel locally and within the states where licensed.
• Establish trusted relationships to enhance health outcomes.
• Refer members to health plans and encourage the involvement of their loved ones.
• Inform primary care physicians about members’ health statuses and quality of life.
• Possession of an active, unrestricted license(s) in relevant coverage area(s).
• Capability to acquire and maintain multi-state licenses in required coverage areas.
• MD or DO degree.
• Competence to safely perform duties in homes with household pets, secondhand smoke, or airborne allergens and irritants.
• Willingness to travel locally and within licensed states.
• Availability to travel up to 100% of the time within a specified coverage area.
• Possible need for overnight hotel stays, car rentals, and/or flights.
• Ability to conduct virtual visits as needed.
