In-Home Traveler Nurse Practitioner, Full Time

This is a fully remote position, open to applicants in District of Columbia.

📋 Description

• Conduct in-home health assessments in the residences of members

• Interact with individuals to help close gaps in care

• Travel primarily within a designated coverage area, potentially up to 100% of the time

• Carry out approximately 45-minute appointments

• Review patients' medication and medical histories

• Perform physical assessments

• Execute additional diagnostic tests as ordered, such as diabetic eye exams, spirometry, and bone density screenings

• Utilize an iPad and clinical workflow for each assessment

• Conduct virtual appointments as necessary

• Offer Diagnostic & Preventive Services products

• Establish trustworthy relationships and assist in enhancing health outcomes

• Visit individuals nationwide and connect them to the appropriate care

• Travel locally and within licensed states, which may include overnight hotel stays, car rentals, and flights

⛳️ Requirements

• Active and unrestricted license(s) within coverage area(s)

• Board certification (mandatory)

• Capable of obtaining and maintaining multi-state licenses in the coverage area, as necessary

• Must be able to cover all counties within the state and adapt to other business needs

• Must be able to safely and comfortably perform duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants

• Qualification as a Nurse Practitioner or Physician Assistant

• Willingness and capacity to travel locally and within licensed states, potentially up to 100% of the time

• Capability to conduct virtual visits as required

• Proficient in using an iPad and clinical workflow

• Competent in performing medication and medical history reviews, physical evaluations, and diagnostic tests as ordered
