In-home Traveler – NP or PA

This is a fully remote position, open to applicants in New Hampshire.

📋 Description

• Deliver in-home health assessments to members at their residences

• Travel predominantly within a specified coverage area, potentially up to 100% of the time

• Establish trusted relationships with health plan members to enhance health outcomes

• Conduct visits lasting approximately 45 minutes

• Address health inquiries and evaluate medications and medical histories

• Execute physical evaluations

• Carry out additional diagnostic tests as requested by the health plan, such as diabetic eye exams and spirometry

• Utilize an iPad and a user-friendly clinical workflow for each evaluation

• Perform virtual visits when necessary

• Offer Signify Health services, including Diagnostic & Preventive Services products

• Travel throughout all counties in New Hampshire and adapt to other areas based on business requirements

• Facilitate connections to appropriate care

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualifications

• Active, unrestricted licenses in relevant coverage areas

• Board certification is mandatory

• Capability to obtain and maintain multi-state licenses as needed

• Willingness to cover all counties in New Hampshire and adapt to other business needs

• Comfortably and safely perform duties in homes that may have household pets, secondhand smoke, airborne allergens, and other irritants

• Ability to travel locally and within licensed states, up to 100% of the time

• Readiness to stay overnight in hotels and utilize car rentals and/or flights as necessary

• Competence to conduct virtual visits when required

• Ability to provide diagnostic and preventive services, including spirometry and diabetic eye exams

• Proficiency in using an iPad and clinical workflow
