In-Home Physician

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct part-time In-Home Health Evaluations at members’ residences.

• Perform visits lasting approximately 45 minutes.

• Interact individually with members and address health-related inquiries.

• Assess medications and review medical histories.

• Execute physical evaluations.

• Carry out additional diagnostic tests as prescribed by the health plan.

• Utilize an iPad and clinical workflow for each evaluation.

• Assist in identifying care gaps and linking members to suitable care.

• Travel locally and across licensed states as required.

• Conduct virtual visits when necessary.

• Deliver Signify Health services, including Diagnostic & Preventive Services.

• Establish trusted relationships to enhance health outcomes.

⛳️ Requirements

• MD or DO degree required.

• Must possess an active, unrestricted license(s) in the relevant coverage area(s).

• Flexibility to travel locally and within licensed states as needed.

• Capability to perform duties comfortably and safely in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Multi-state licenses are preferred.

• Ability to carry out virtual visits as required.

• Competence in providing Diagnostic & Preventive Services, which includes spirometry, diabetic eye exams, and bone density screenings.
