In-Home Nurse Practitioner, Physician Assistant – Part Time

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct in-home health assessments for members of health plans.

• Carry out visits that last approximately 45 minutes.

• Interact directly with members to address their health inquiries.

• Review patients' medications and medical histories.

• Execute physical assessments.

• Administer additional diagnostic tests as required, such as diabetic eye examinations and spirometry.

• Utilize an iPad and follow clinical workflows for each assessment.

• Travel locally and across licensed states as necessary.

• Provide virtual consultations when required.

• Offer Signify Health services, including Diagnostic & Preventive Services like spirometry, diabetic eye exams, and bone density screenings.

• Establish trusted relationships and contribute to enhancing health outcomes.

• Identify gaps in care and facilitate connections for members with appropriate healthcare services.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as necessary.

• Capability to conduct virtual visits as required.

• Competence to safely perform duties in homes that may have household pets, secondhand smoke, airborne allergens, or irritants.

• Qualification as a Physician or Physician Assistant as implied by the title and specified requirements of the role.

• Proficient in using an iPad and following clinical workflows for evaluations.
