In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct in-home health assessments for members of health plans.

• Interact with individuals in their residences to help close care gaps.

• Perform visits lasting around 45 minutes, which include reviewing medication, medical history, and conducting physical evaluations.

• Administer additional diagnostic tests as requested by the health plan, such as diabetic eye examinations and spirometry.

• Utilize an iPad along with a clinical workflow for each assessment.

• Foster trusted relationships to enhance health outcomes.

• Make visits to health plan members across the nation as part of Signify Health’s care initiatives.

• Collaborate with Signify Health as a clinical partner.

• Travel locally and within authorized states as necessary.

• Conduct virtual consultations when required.

• Deliver Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density assessments.

• Possession of an active, unrestricted license(s) in applicable coverage areas.

• Board certification is mandatory.

• Multi-state licenses are advantageous.

• Capability to travel locally and within authorized states as necessary.

• Ability to perform visits in private homes that may have household pets, secondhand smoke, airborne allergens, or irritants.

• Comfort and safety in executing clinician responsibilities in diverse home settings.
