This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct health evaluations in the homes of members.

• Foster relationships with individuals to enhance health outcomes.

• Carry out visits lasting approximately 45 minutes, which include reviewing medication and medical histories as well as performing physical evaluations.

• Execute additional diagnostic and preventive services as requested, including diabetic eye examinations, spirometry, and bone density screenings.

• Utilize an iPad and follow clinical workflows during each evaluation.

• Travel locally and within authorized states, potentially covering all counties in a state and other necessary business areas.

• Perform virtual visits when required.

• Support individuals nationwide in connecting them to suitable care.

• Provide educational resources and address health-related inquiries from members.

• Collaborate as a clinician with Signify Health.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Capable of obtaining and maintaining multi-state licenses in the coverage area as needed.

• Must be able to cover all counties within the state and adapt to other business needs.

• Comfortable and safe performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Able to travel locally and within licensed states.

• Willingness and capability to travel up to 100% of the time, including possible overnight stays, car rentals, and/or flights.

• Proficient in conducting virtual visits as necessary.

• Competent in using an iPad and clinical workflows.

• Ability to perform medication and medical history reviews, physical evaluations, and necessary diagnostic tests.
