In Home Health Physician – Part Time

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver part-time in-home health assessments to members of health plans.

• Conduct visits lasting approximately 45 minutes that include reviews of medication, medical history, and physical evaluations.

• Carry out additional diagnostic tests as directed by the member’s health plan, such as diabetic eye examinations, spirometry, and bone density assessments.

• Foster trustworthy relationships and contribute to enhancing health outcomes.

• Recognize care gaps and link members to suitable healthcare services.

• Perform virtual visits as necessary.

• Provide Signify Health Diagnostic & Preventive Services products.

• Travel locally and within licensed states as required.

• Primarily work in the private homes of members.

⛳️ Requirements

• MD or DO degree is required.

• Must hold active, unrestricted licenses in relevant coverage areas.

• Willingness to travel locally and within licensed states as needed.

• Capability to conduct virtual visits as necessary.

• Ability to safely and comfortably perform duties in private residences that may have household pets, secondhand smoke, airborne allergens, or irritants.

• Multi-state licenses are preferred.

• Proficiency in using an iPad and clinical workflows for evaluations.
