I/DD Care Manager – QP

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Act as the main point of contact and guide for members with Intellectual and Developmental Disabilities (I/DD) and/or those with dual diagnoses.

• Direct Team-Based Care that combines physical health, behavioral health, and long-term support services.

• Engage with members in their communities to evaluate their current and future needs.

• Develop Person-Centered Care Plans and Individual Support Plans (ISP).

• Organize interdisciplinary team meetings that include doctors, specialists, providers, and families.

• Assist members during transitions, such as moving from school to adulthood and returning home from care facilities.

• Help members achieve maximum independence and locate paid support options.

• Inform members and their families about their rights.

• Connect members with necessary services and the provider network.

• Travel frequently to fulfill job responsibilities.


⛳️ Requirements

• Bachelor’s degree in a human services discipline such as psychology, social work, nursing, or a related field, along with a minimum of 2 years of full-time experience working with individuals with Intellectual and Developmental Disabilities.

• Alternatively, a bachelor's degree in a non-human services field plus at least 4 years of full-time experience working with individuals with Intellectual and Developmental Disabilities.

• Or a master’s degree with at least 1 year of full-time experience working with individuals with Intellectual and Developmental Disabilities.

• A minimum of 2 years of experience working with individuals with significant Long-Term Services and Supports (LTSS) needs due to their disabilities in a community care setting.

• Must reside in North Carolina or within 40 miles of the NC border.

• Willingness to travel regularly as required to carry out job duties.

• Knowledge of, or eagerness to master, Medicaid regulations, 1915i services, and the Tailored Plan framework.

• Proficiency in person-centered planning and care management.
