Hospice Triage RN, PT

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Offer telephone triage services to hospice patients and their families.

• Evaluate patient conditions and decide on the appropriate next steps.

• Work collaboratively with on-call teams to ensure coordinated care and resource allocation.

• Document all communications and interventions accurately.

• Maintain a composed and professional attitude while managing urgent calls.

• Assist in the delivery of after-hours hospice care and ensure timely interventions.

⛳️ Requirements

• Possess an active multistate Registered Nurse (RN) license.

• Previous experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Capable of typing documentation and notes efficiently in a fast-paced setting.

• Able to manage stress and multitask effectively while answering calls.

• Strong skills in communication and critical thinking.

• Ability to work independently in a remote setting.

• Consistent attendance, proactive communication, and reliable internet connection during all scheduled shifts.

• Scheduled work hours: 3:30 PM to 11 PM with rotating shifts on Saturdays and Sundays from 7:30 AM to 4 PM CST.
