Healthcare Advocate

This is a fully remote position, open to applicants in United States.

📋 Description

• Establish trustworthy relationships with patients by demonstrating active listening, respect, and transparency.

• Evaluate medical, social, and environmental aspects to uncover barriers and identify needs.

• Develop personalized care plans that incorporate medical, social, and behavioral resources.

• Connect patients to community services, such as transportation, meal delivery, home care, and social support.

• Review medical bills and provide assistance with claim appeals or clarifications.

• Identify opportunities for medication cost savings, co-pay assistance, and formulary options.

• Link patients to caregivers, community programs, peer support groups, and other social support networks.

• Organize and maintain comprehensive medical records, care histories, and communications with providers.

• Coach and assist patients in managing their conditions and navigating their care effectively.

• Accurately document interactions, observations, and patient progress in Baba's systems in a timely manner.

• Collaborate with clinical, product, and operations teams to enhance workflows and address complex issues.

• Offer constructive feedback on platform features, tools, and processes to enhance Baba's care copilot capabilities.

⛳️ Requirements

• Associate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, Human Services, or a related healthcare field, OR a Certificate/Diploma in Practical Nursing (LVN/LPN).

• Preferred active credentials include RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management qualifications.

• A minimum of 2 years of experience in care navigation, patient advocacy, care management, case management, home health, discharge planning, utilization management, or clinical social work.

• Experience in supporting older adults or Medicare populations is highly desirable.

• Strong understanding of Medicare regulations, eligibility, benefit structures, claims, and appeals processes.

• In-depth knowledge of Social Determinants of Health (SDOH).

• Experience working with diverse and underserved communities.

• Excellent communication skills with the ability to simplify complex concepts.

• Proficiency in using digital tools, EMRs/care-management software, and adapting to new technologies.

• Highly organized, proactive, and capable of managing multiple priorities without compromising quality.

• Legally eligible to work in the United States.
