Float Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Act as a Float Case Manager for participants in Wisconsin's Family Care program.

• Manage caseloads for colleagues in the SW Pod 2 region who have left their position or are on leave.

• Evaluate members' health and safety requirements, strengths, preferences, and interests.

• Create detailed Member Care Plans (MCPs).

• Collaborate with a Field Care Nurse to deliver services in accordance with each MCP.

• Perform face-to-face social assessments during enrollment and at least every six months.

• Execute quarterly in-person visits and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure cost-effective service provision.

• Assess risk factors and provide education to members.

• Keep accurate case notes, service authorizations, and updates to MCPs.

• Travel to members' locations approximately 40% of the time.

• Travel across the southwest Wisconsin region, with occasional visits to Humana offices for training or meetings.

⛳️ Requirements

• A four-year bachelor's degree in Human Services or a related discipline and one (1) year of experience with at least one Family Care target population, OR a four-year bachelor's degree in another field and three (3) years of relevant experience with at least one Family Care target population.

• A minimum of three years of experience within the Family Care system.

• Must live in or near the southwest region of Wisconsin.

• A valid state driver's license is required.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• Completion of tuberculosis (TB) screening is required if selected.

• Home internet speed must be at least 25 Mbps download and 10 Mbps upload.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information.
