
Field Service Coordinator

This is a fully remote position, open to applicants in United States.
β’ Assess and evaluate the needs and requirements of members.
β’ Meet with members at their locations to assess needs and identify barriers.
β’ Connect members and their families with suitable care, wellbeing, and community resources.
β’ Conduct ongoing long-term services and support (LTSS) assessments utilizing person-centered approaches.
β’ Reach out to members via telephone and/or in person to establish goals and priorities.
β’ Evaluate resources and devise plans of care.
β’ Identify LTSS providers and establish community partnerships.
β’ Develop and adjust service plans in collaboration with relevant care team members.
β’ Assist members in navigating LTSS, environmental, social, housing, educational, and other services.
β’ Utilize available member information to prevent redundant assessments.
β’ Help members in maintaining their Medicaid eligibility.
β’ Collaborate with the Medical Director, Geriatrician, and Care Coordinator to ensure cohesive service delivery and positive outcomes.
β’ Bachelor's degree in a health and human services field.
β’ A minimum of 2 years of relevant experience.
β’ Proficiency in Microsoft Office Word and Excel.
β’ Outstanding verbal and written communication skills, along with strong interpersonal abilities.
β’ Capability to utilize various electronic information applications and software programs.
β’ Completion of tuberculosis (TB) screening is required if selected.
β’ A valid state driver's license.
β’ Personal vehicle liability insurance meeting state minimum requirements or specified Humana limits, whichever is greater.
β’ Must have a designated room with a locked door that can function as a home office.
β’ Minimum home internet speed of 25 Mbps for download and 10 Mbps for upload.
β’ Licensed RN or Licensed Masterβs Social Worker is preferred.
β’ Previous experience with Medicare and Medicaid recipients is preferred.
β’ Familiarity with electronic case note documentation and various computer applications/systems is preferred.
β’ Experience in health promotion, coaching, and wellness is preferred.
β’ Knowledge of community health and social service agencies, as well as community resources, is preferred.
