Field Case Manager

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Engage directly with injured workers and medical professionals.

• Promote advocacy, progressive medical interventions, and timely reintegration into the workplace.

• Serve as an intermediary and ensure effective communication among all stakeholders.

• Oversee complex medical issues, treatment planning, and recovery from health challenges.

• Act as a patient advocate for injured workers, their families, and caregivers.

• Direct and coordinate care throughout the recovery journey.

• Aid injured workers in achieving recovery and independence through advocacy, communication, education, resource identification, and service facilitation.

• Identify suitable providers and facilities across the service continuum.

• Ensure that resources are utilized efficiently and cost-effectively to maximize value for both the client and the reimbursement source.

• Maintain a home-based office while frequently traveling.

⛳️ Requirements

• Must reside in or near the Baton Rouge, Lafayette, LA area.

• Knowledge and experience in medical/clinical or rehabilitation fields.

• A graduate degree in a health or human services discipline is required, along with one of the following certifications: CCM, CDMS, or CRC.

• RN licensure is preferred.

• A valid driver's license is necessary.

• High-speed internet access is required.

• Frequent travel is necessary.

• Ability to interact face to face with injured workers and medical providers.

• Capability to support advocacy, medical treatment planning, communication, education, and service facilitation.
