Field Care Manager – Behavioral Health

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Assess members' needs and requirements to support optimal wellness.

• Direct members and their families to suitable care and wellbeing resources.

• Oversee members' physical, environmental, and psychosocial health challenges.

• Address obstacles that impede care delivery.

• Perform assessments and evaluations to ensure patients are progressing towards their desired outcomes.

• Develop member care plans as necessary.

• Comprehend departmental, segmental, and organizational strategies and operational objectives.

• Report directly to the Manager of Care Management Behavioral Health.

• Engage in mandatory fieldwork and travel up to 50% of the time.

⛳️ Requirements

• Bachelor's degree in a health or human services discipline.

• A minimum of 2 years of relevant experience.

• Proficiency in Microsoft Office Word and Excel.

• Outstanding verbal and written communication as well as interpersonal skills.

• Capability to utilize various electronic information applications and software programs.

• Required TB screening for this patient-facing position.

• Valid driver's license from the state.

• Personal vehicle liability insurance that meets minimum requirements.

• A separate room with a locked door designated as a private home office.

• Minimum home internet speed of 25 Mbps for downloads and 10 Mbps for uploads.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information.

• Preferred: experience with electronic case note documentation and multiple computer applications or systems.

• Preferred: experience working with Medicare and Medicaid recipients.

• Preferred: background in health promotion, coaching, and wellness.

• Preferred: familiarity with community health and social service agencies and local resources.
