This is a fully remote position, open to applicants in Illinois.
• Provide comprehensive behavioral health care management for members enrolled in Illinois Medicaid.
• Act as the primary contact for members through both in-person and telephonic interactions.
• Offer personalized care coordination, crisis intervention, and peer support services.
• Assist members facing behavioral health challenges, including Severe Mental Illness (SMI), Serious Emotional Disturbance (SED), Substance Use Disorder (SUD), and those involved with the justice system.
• Engage with members in their communities and perform in-person assessments.
• Coordinate behavioral health and medical services while ensuring adherence to treatment plans.
• Enhance health literacy and address social determinants of health.
• Lead interdisciplinary care teams and manage care planning, transitions, and service delivery.
• Facilitate meetings of interdisciplinary care teams involving providers, Service Coordinators, and Care Management Extenders.
• Assist with transitions of care.
• Collaborate with internal departments, healthcare providers, and community organizations.
• Adhere to regulatory compliance processes as dictated by IDHS, CMS, and NCQA.
• Travel up to 25% within Illinois for collaboration, meetings, and field engagements.
• Candidates must reside in Cook or Kane County, Illinois, within specified ZIP codes or within a 10-mile radius.
• Must hold an active Illinois license in LCSW, LMFT, or LCPC; provisional licenses or supervisees are not eligible.
• A minimum of 2 years of post-degree clinical experience in a behavioral health environment is required.
• Experience in case management with complex populations including SMI, SUD, and SED is essential.
• Capability to travel to regional facilities and homes for in-person assessments.
• Proficient in using electronic information applications/software, including electronic medical records.
• Intermediate to advanced computer skills are necessary.
• Familiarity with Microsoft Word, Outlook, and Excel is required.
• A valid driver's license, car insurance, and reliable transportation are mandatory.
• Availability to work Monday–Friday, from 8:00 a.m. to 5:00 p.m. Central Standard Time.
• Must meet remote work internet requirements: at least 25 Mbps download and 10 Mbps upload speeds.
• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information is essential.
• Tuberculosis screening must be completed if selected for the role.
• Personal vehicle liability insurance must be maintained under the driver safety program.
• $5,000 hiring bonus: $2,500 after 6 months and another $2,500 after 1 year of continued employment.
