Field Care Manager, Behavioral Health

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Evaluate and assess the needs and requirements of Medicaid members.

• Develop individualized care plans and coordinate necessary services.

• Ensure that members receive services in the least restrictive environments.

• Plan and execute interventions to address care requirements.

• Monitor and assess case management plans relative to members' personal objectives.

• Support members and their families in navigating appropriate care resources.

• Facilitate communication with payer sources, providers, interdisciplinary teams, and others involved in the care of members.

• Conduct home visits for Medicaid members in various settings such as assisted living and long-term care facilities.

• Perform face-to-face assessments that require 75–90% local travel.

• Identify and overcome obstacles to effective care delivery.

• Continuously oversee patient care through assessments, data collection, conversations with members, and proactive care planning.

⛳️ Requirements

• Candidates must reside in Cook or Will County, Illinois, within designated ZIP codes or within a 10-mile radius.

• Must hold an active Illinois license in LCSW, LMFT, or LCPC; no supervisees or provisional licenses accepted.

• A minimum of 2 years of post-degree clinical experience in a behavioral health environment is required.

• Experience in case management with complex SMI, SUD, and SED populations is essential.

• Willingness to travel to regional facilities and homes for face-to-face assessments.

• Proficiency in using electronic information applications and software programs, including electronic medical records.

• Intermediate to advanced computer skills are necessary.

• Familiarity with Microsoft Word, Outlook, and Excel is required.

• A valid driver's license, car insurance, and dependable transportation are mandatory.

• TB screening must be completed if selected for the position.

• Candidates must maintain personal vehicle liability insurance that meets specified limits.

• Access to home internet with a minimum speed of 25 Mbps download and 10 Mbps upload is required.

• A dedicated workspace free from interruptions is necessary to safeguard PHI/HIPAA information.
