Field Care Manager

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Evaluate and assess the needs and requirements of members to achieve or maintain optimal wellness.

• Direct members and their families to suitable care and wellness resources.

• Facilitate interactions between members, families, and care resources.

• Address members’ physical, environmental, and psychosocial health challenges using a variety of strategies and techniques.

• Identify and overcome barriers to effective care.

• Continuously monitor patient care through ongoing assessments and evaluations.

• Ensure that patients make progress towards their desired outcomes.

• Develop member care plans as necessary.

• Interpret the strategies and operational objectives of the department, segment, and organization.

• Make independent decisions regarding work methods, especially in situations that are unclear.

• Adhere to established guidelines and procedures.

⛳️ Requirements

• LCSW, LPC, LAC, or LMFT accreditation.

• Experience with the Department of Children and Family Services.

• Relevant State licensure in the field of study.

• A strong passion for contributing to an organization that is dedicated to enhancing consumer experiences.

• Experience in Managed Care (preferred).

• Bilingual skills (preferred).

• Completion of tuberculosis (TB) screening is required if selected.

• A minimum home internet download speed of 25 Mbps.

• A minimum home internet upload speed of 10 Mbps.

• A designated workspace free from ongoing interruptions to protect member PHI/HIPAA information.
