FCP Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver care management services via the Family Care Partnership program

• Perform psychosocial and thorough assessments

• Employ Resource Allocation Decision-making methodology to determine preferred outcomes and strategies

• Identify community resources and cost-effective services or equipment

• Create, coordinate, and oversee member service plans and resources

• Conduct regular reassessments and monitor members' health and safety

• Assist with complaints, grievances, and advocacy services

• Document member details, contacts, care, and services in accordance with procedures and audit requirements

• Foster working relationships with members, families, and staff from external agencies

• Collaborate with physicians and healthcare professionals to ensure coordinated care

• Help resolve conflicts between members and service providers

• Educate members and providers regarding benefits, service providers, protocols, and medical service utilization

• Provide referrals for community resources and social services

• Monitor treatment engagement and identify alternatives or solutions in collaboration with staff, members, and providers

⛳️ Requirements

• Social Worker certified in Wisconsin with at least one (1) year of experience working with a Family Care target group; OR a 4-year bachelor's degree or higher in the Human Services field and a minimum of one (1) year of experience with a Family Care target population; OR a 4-year bachelor's degree or higher in another field with at least three (3) years of experience working with a Family Care population

• Familiarity with community resources and long-term care options

• Capability to travel throughout Milwaukee County to perform assessments in members' homes

• Valid state driver's license

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100

• Required TB screening for selected candidates

• Ability to meet work-from-home internet requirements: a minimum of 25 Mbps download and 10 Mbps upload

• Capacity to work from a dedicated space that ensures the protection of member PHI/HIPAA information
