Engagement Specialist, Experienced

This is a fully remote position, open to applicants in Pennsylvania, +1 more state.

📋 Description

• Act as the primary point of contact for patients through onboarding, appointment scheduling, and relationship-driven outreach.

• Assist in the completion of CHEs in nephrology offices, clinics, or via telehealth services.

• Collaborate with Nurse Practitioners and clinics to prepare for face-to-face appointments.

• Conduct outbound calls to patients, caregivers, and clinics for scheduling, reminders, and rescheduling of CHEs.

• Arrange telehealth CHEs for patients who are unable to attend in person.

• Greet new patients and gather concerns from current patients.

• Elevate concerns to RNs or Nurse Practitioners as necessary.

• Ensure accurate documentation and relay patient updates to the care team.

• Foster trust, encourage patient participation, and address engagement challenges.

• Prioritize outreach based on urgency and significance to enhance patient impact.

• Operate across both ES and CC roles.

⛳️ Requirements

• High School Diploma or GED is mandatory.

• An Associate’s or Bachelor’s Degree is preferred.

• Previous IKC experience is required (CC/ES).

• A minimum of 2 years of professional experience in a customer service, patient-facing, or telephonic outreach role is preferred.

• Experience in a healthcare environment is preferred.

• Intermediate skills in Microsoft Word, Excel, PowerPoint, and Outlook are necessary.

• Excellent verbal and written communication abilities.

• Skills in active listening and motivational techniques.

• Highly organized with outstanding time management and follow-through capabilities.

• Capacity to work independently as well as in collaboration with care team members.

• Residency in Pacific or Mountain time zones is required.

• A quiet, private home workspace equipped with high-speed internet is essential.
