This is a fully remote position, open to applicants in District of Columbia, +3 more states.
• Deliver extensive program and project management support for the design, launch, management, evaluation, and scaling preparation of the Elevated Risk Care Management (ERCM) Pilot.
• Assist Government program leaders and designated stakeholders throughout the Concept, Explore, Design, Implement, Evaluate, and Spread phases.
• Create and implement integrated project plans, schedules, milestones, dependencies, as well as tracking risks, issues, decisions, and action items.
• Establish project governance structures, including Integrated Project Team frameworks, meeting schedules, decision-making protocols, and escalation pathways.
• Coordinate cross-functional workstreams and gather stakeholder input across clinical and operational teams.
• Develop ERCM operating models, workflows, roles, responsibilities, escalation processes, implementation strategies, standard operating procedures, training resources, and communications.
• Oversee implementation at three selected pilot sites, which includes onboarding, readiness, training, workflow execution, and stakeholder engagement.
• Set up reporting processes, dashboards, and performance-monitoring systems.
• Assist in evaluation through data collection, organization, interpretation, performance measurement, reporting, and assessment of pilot effectiveness.
• Formulate recommendations, transition documents, implementation toolkits, and resources for phased national expansion.
• Facilitate meetings, working sessions, stakeholder engagements, and program reviews; prepare agendas, briefings, minutes, decision logs, and action trackers.
• Draft technical, operational, executive-level, and evaluation documents while ensuring version control of project documentation.
• Master’s degree in healthcare administration, public health, business administration, health services, social sciences, clinical discipline, or a related field.
• At least 10 years of pertinent healthcare experience.
• Project Management Professional (PMP) certification.
• Minimum of six years of experience supporting a large corporation, healthcare organization, or government agency.
• Preferred experience in supporting the Department of Veterans Affairs (VA) or another large federal healthcare system.
