ECM Territory Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Lead a team of up to 15 Community Health Navigators to accomplish monthly and quarterly performance objectives.

• Supervise effective outreach and member engagement through door-to-door visits and telephone outreach.

• Ensure timely completion of assessments and care plans, provision of quality care, and successful member graduation.

• Maintain audit readiness and compliance with HIPAA, documentation standards, and regulatory requirements from the state and health plans.

• Establish clear expectations, demonstrate effective behavior, and foster open communication.

• Support field teams through time spent in the field and engagement with community organizations, health plans, and other partners.

• Educate and mentor the team on escalation protocols, panel management, safety expectations, and providing exceptional member care.

• Remain available for on-call responsibilities during the scheduled workweek and adjust the schedule as necessary.

• Achieve team performance metrics, objectives, and deadlines while inspiring staff.

• Oversee daily territory operations, including individual performance and caseload reviews, and tracking productivity, growth, member engagement, quality of care, and graduation.

• Deliver ongoing virtual and field-based coaching and professional development.

• Provide community-based opportunity resources to the Community Impact Partnership Program Director.

• Guide Community Health Navigators on SMART goals, interventions, and strategies for overcoming member barriers and objections.

• Proactively and creatively address issues raised by Community Health Navigators.

• Assist in program expansion, staffing needs, and operational improvements.

• Recruit, manage, and coach staff.

• Oversee Profit and Loss for the territory.

• Ensure timely review and approval of payroll, expenses, and other compliance requirements for the territory.

⛳️ Requirements

• A minimum of 3-4 years of experience managing personnel in health, social, or community services.

• High School diploma or equivalent.

• Strong analytical abilities with a solid understanding and comfort level with data, dashboards, and reporting tools.

• Capability to manage a Profit and Loss statement.

• Ability to adapt to ongoing changes and perform effectively under pressure in a fast-paced and demanding environment.

• Proficient in problem solving, critical thinking, and effective collaboration.

• Understanding of patient-centered care and whole person care models.

• Capability to meet the physical demands of the role, including extended periods of stationary desk work, constant operation of computers and office equipment, occasional movement within the workspace, lifting objects up to 10 pounds, and close-range vision for computer tasks and small text.
