Community Based, Case Management RN

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Conduct assessments for clients

• Execute nursing delegations

• Instruct clients and caregivers on nursing procedures

• Supervise clients residing at home

• Advocate on behalf of clients

• Schedule appointments autonomously

• Manage an independent caseload

• Work with elderly and medically vulnerable clients in their residences

• Provide limited hands-on care

• Collaborate with clients to support their care needs without after-hours calls or weekend commitments

⛳️ Requirements

• Registered Nurse licensed in Oregon

• RN license held for a minimum of 3 years

• At least 3 years of experience in teaching nursing tasks and performing delegations

• Background in case management and delegations

• No restrictions on nursing practice

• Must pass background checks and drug screenings

• Current Oregon driver's license in good standing

• Preference for Home Health Care experience

• Self-driven, friendly, and positive attitude

• Enjoys working with elderly and medically fragile clients
