
Clinical Services Follow-Up Coordinator

This is a fully remote position, open to applicants in Oklahoma.
• Offer follow-up support, outreach for consumer satisfaction, service linkage, and advocacy assistance.
• Conduct outbound follow-up calls with individuals referred through designated agency programs and services.
• Evaluate the needs of callers and clients, connecting them with suitable providers.
• Execute continuous follow-up in accordance with established guidelines.
• Keep precise documentation of follow-up activities, service outcomes, and client information.
• Prepare and submit reports on a weekly, monthly, quarterly, and annual basis.
• Provide training and support regarding follow-up processes and procedures.
• Adhere to Follow-Up Quality Improvement protocols and assist in enhancement efforts.
• Offer additional quality improvement support when scheduling or departmental needs arise.
• Remain updated on research, best practices, and program developments related to suicide prevention, crisis response, information and referral, Tribal services, and other agency initiatives.
• Participate in suicide prevention committees both within and outside of HeartLine.
• Assist with suicide prevention presentations as requested.
• Advocate at community, state, and national levels to contribute to the reduction of suicide rates.
• Perform other assigned duties.
• Some college experience is required.
• A Bachelor’s degree in social services, psychology, social work, or a related field is preferred.
• Experience in the nonprofit sector is preferred.
• A background in mental health or crisis intervention is preferred.
• Relevant experience may be considered in lieu of formal educational qualifications.
• Strong verbal and written communication skills are necessary.
• Excellent interpersonal skills and a commitment to customer service are essential.
• Ability to collaborate with staff, volunteers, and team members while working independently.
• Proficient computer literacy and capability to use multiple digital systems.
• Effective time management and prioritization skills are required.
• A compassionate and nonjudgmental approach to individuals with diverse and complex needs is important.
• Strong attention to detail and the ability to multitask are essential.
• Willingness to work days, evenings, and weekends is necessary.
• Ability to manage stress related to the nature and type of chats received.
• Physical capability to lift 20 pounds, sit for extended periods, talk on the telephone, and bend/stoop.
• Must reside in Oklahoma.
