Clinical Pharmacist – CareKinesis Clinical Initiatives

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Perform thorough and focused Medication Safety Reviews for PACE participants utilizing proprietary clinical platforms.

• Create personalized, measurable, and goal-oriented therapeutic care plans.

• Conduct medication reviews and reconciliations for new enrollments, annual assessments, significant changes, care transitions, and high-risk clinical events.

• Identify, assess, document, and facilitate the resolution of medication-related issues.

• Collaborate with healthcare providers, interdisciplinary team members, pharmacists, and other medical professionals.

• Communicate recommendations for medication optimization, deprescribing, and goals of care.

• Engage with providers through medication safety reviews, clinical consultations, polypharmacy assessments, fall-risk evaluations, and care-transition reviews.

• Track and document outcomes and progress towards therapeutic objectives.

• Monitor participants on high-risk medications or those managing complex chronic disease regimens.

• Take part in client meetings, interdisciplinary discussions, P&T Committee meetings, drug-utilization reviews, quality improvement initiatives, and educational programs.

• Ensure compliance with pharmacy regulations, accreditation standards, HIPAA, and requirements related to Fraud, Waste, and Abuse, along with patient-safety reporting.

• Travel may be necessary for organizational events, team meetings, trade shows, conferences, and client engagements.

⛳️ Requirements

• Graduate from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE), or an equivalent pharmacy program outside the U.S.

• Current, active license/registration in good standing with the state board of pharmacy where practice will be conducted.

• Active Board of Pharmacy Specialties certification in geriatrics (BCGP) is preferred.

• Prior direct contact experience with prescribers and patients is preferred.

• Experience or a strong interest in clinical pharmacogenomics is preferred.

• Above-average understanding of geriatric pharmacotherapy.

• Capability to apply knowledge of geriatric pharmacotherapy to patients with concurrent illnesses and multiple medications.

• Ability to resolve complex medication-related challenges.

• Strong analytical skills, clinical judgment, and decision-making capabilities.

• Proficient technical and computer skills, including familiarity with pharmacy software.

• Proven problem-solving, conflict-resolution, and project-management abilities.

• Excellent written, verbal, and interpersonal communication skills.

• Detail-oriented and organized; capable of multitasking, working under pressure, and meeting deadlines.

• Knowledge of state and federal laws and regulations, including HIPAA and Fraud, Waste, and Abuse.

• Availability to work an average of 40 hours per week, with the potential for ad-hoc work outside normal business hours.
