
Clinical Pharmacist, Bilingual

This is a fully remote position, open to applicants in United States.
• Conduct outreach to patients and handle incoming calls to deliver pharmacist services.
• Provide medication therapy management services, including Comprehensive Medication Reviews.
• Evaluate medication appropriateness to enhance therapy and improve clinical outcomes.
• Execute comprehensive medication reviews, targeted medication reviews, medication reconciliation, prior authorization review, and other clinical services.
• Perform personalized asynchronous video consultations and patient check-ins.
• Reinforce clinical information, convert patients into clinical programs, and address patient needs.
• Assess unnecessary medications, identify additional medication needs, inappropriate dosing, adverse drug reactions, ineffective medications, and adherence issues.
• Document clinical recommendations, therapy responses, and patient care notes.
• Communicate clinical recommendations to provider offices and follow up to ensure implementation.
• Collaborate with nurses, health coaches, management, product managers, and client-facing staff.
• Meet productivity and performance standards.
• Create or revise operational policies and procedures for pharmacy services.
• Enhance and develop processes and workflows.
• Provide guidance to Product and Experience teams regarding product improvements.
• Hire, train, mentor, counsel, and evaluate direct reports as necessary.
• Active Pharmacist License with a minimum of 3 years in pharmacy practice.
• Medically fluent in both English and Spanish.
• Capability to obtain licensure in additional states as required.
• Must effectively communicate with the elderly population in a clear and fluid manner via telephone.
• Excellent communication skills with the ability to establish rapport with patients.
• Outstanding listening and interpersonal skills.
• Ability to work autonomously as well as collaboratively within a virtual/decentralized team.
• Capacity to maintain confidentiality of information.
• Self-motivated with the ability to manage multiple tasks.
• Proficient in Microsoft Teams, Outlook, Word, Excel, and other relevant software.
• Must be able to attend meetings and interact with patients via video.
• Native fluency in Spanish is preferred.
• Experience in digital health programs and/or with a health startup is preferred.
• Background in working with hospitals or health plans on quality improvement is preferred.
• Proficiency in Salesforce and other documentation platforms is preferred.
• Sales or call center experience is preferred but not mandatory.
• Must be eligible to work in the United States; visa sponsorship is not available.
