Clinical Outreach Specialist

This is a fully remote position, open to applicants in Arizona, +9 more states.

📋 Description

• Interact with potential clients and document all communications in the Client Relationship Management (CRM) system.

• Conduct 60-100 outbound calls each day to engage with prospective clients.

• Inform prospective clients about Enhanced Care Management (ECM) services.

• Support client enrollment by evaluating needs and connecting clients with the care management team.

• Relay crucial information and client preferences to the care team for tailored care planning.

• Help clients complete psychosocial assessments and formulate care plans with specific, actionable goals.

• Review medical records to identify client needs and suggest suitable interventions.

• Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.

• Attend weekly Team Operations Meetings to enhance coordination and care delivery.

⛳️ Requirements

• Availability to work Monday through Friday from 8:30am to 5:00pm Pacific Time, with some flexibility for evenings and weekends.

• Possession of an active California Licensed Vocational Nurse (LVN) license, or a Medical Assistant certificate from an accredited organization, or a Bachelor's degree in a relevant field.

• At least 2 years of experience in telephonic outreach or enrollment within a high call volume setting.

• Experience with complex care clients and ECM Populations of Focus.

• Strong verbal and written communication abilities.

• Positive interpersonal skills essential for successful client engagement.

• Proficient with technology, including video conferencing, Google Workspace, Slack, and electronic health records.

• Previous experience in a remote position.

• A quiet and secure workspace to maintain member privacy.

• Capability to test internet speed to ensure optimal application performance.

• Bilingual in English and Spanish is highly preferred.

• Candidates must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.

• A background check is required before employment, which may include drug testing or other health screenings based on the role.

• Employees in patient, client, or customer-facing positions must have an influenza vaccination.
