Clinical Operations Nurse – Emergency Department Transitions

This is a fully remote position, open to applicants in Arizona, +1 more state.

📋 Description

• Conduct telephonic patient evaluations and assessments for WellSky’s various payer initiatives.

• Perform telephonic assessments to ascertain the appropriate level of care in compliance with program protocols.

• Engage in solution and program planning meetings.

• Aid in the development of protocols, procedures, and policies.

• Analyze and review patient assessments to identify trends, opportunities, and areas for enhancement.

• Schedule and execute telephonic visits.

• Document assessments and relay follow-up plans within the designated timelines.

• Collaborate with both internal and external stakeholders to ensure prompt and precise patient visits while addressing scheduling requirements.

• Assist in reporting on program progress, outcomes, and key performance indicators.

• Execute other job responsibilities as assigned.

⛳️ Requirements

• A minimum of 2-4 years of relevant work experience.

• Bachelor's degree or equivalent professional experience.

• Active RN Licensure.

• At least 2 years of experience in emergency departments.

• Preferred: Experience in acute/ACO/post-acute care facility care coordination, discharge planning, social services, disease management, population health, home health, and/or post-acute care facilities.

• Willingness to work additional or irregular hours as required.

• Must adhere to applicable security policies and procedures to protect company and client information.

• Must be capable of sitting and viewing a computer screen for extended periods.

• Must be legally authorized to work in the United States.

• Certain client-facing roles may necessitate compliance with immunizations and occupational health mandates.
