Clinical Nurse Coach, RN

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Deliver remote health coaching to individuals at risk for or currently experiencing chronic kidney disease.

• Maintain regular communication with assigned and active members through video, phone, text, and email in English.

• Gather and apply clinical information to create personalized care plans for members and clinician-centered care plans following NCQA standards.

• Evaluate members' health status and their needs for care coordination.

• Collaborate with members' healthcare providers as necessary.

• Employ motivational interviewing techniques, tailored care plans, and the Triple Aim framework to assist members in achieving their health objectives.

• Communicate program advantages to new or inactive members who have chronic kidney disease or are at risk.

• Record, request, and transmit member information in compliance with HIPAA regulations.

• Achieve individual performance metrics that support member engagement and retention.

• Report directly to the Clinical Care Team Manager.

⛳️ Requirements

• Must possess an RN license in good standing and be prepared to obtain licensure in additional states.

• A minimum of five years of RN experience is required.

• Must be willing to work during PST hours and adjust for evenings and weekends as necessary.

• Experience in developing care plans for individuals with chronic conditions and/or Chronic Kidney Disease is strongly preferred.

• BSN is highly preferred.

• CDE or CDCES certification is strongly preferred.

• Experience working 100% remotely as a nurse is highly preferred.

• Must be able and willing to travel occasionally, including overnight stays for corporate meetings, conferences, and health fairs.

• Ability to participate and engage professionally in video meetings.

• Strong technological skills, including proficiency with Microsoft products.

• Self-motivated individual comfortable with uncertainty.

• Innovative mindset with the capability to appropriately question the status quo.

• Excellent written and verbal communication skills.

• Capacity to tackle challenges with a 'yes if...' approach.

• Proficient at managing competing deadlines while maintaining focus on the overall objectives.

• Reliable internet and power supply, with a designated workspace to conduct tasks with minimal distractions.

• Must successfully pass a background and reference check before starting.

• Must be legally authorized to work in the United States without sponsorship.
