Clinical Care Specialist, Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct scheduled telehealth nursing appointments with members managing conditions such as obesity, diabetes, hypertension, and dyslipidemia.

• Execute structured clinical intake and assessments, including medication reconciliation, symptom and side-effect evaluations, and identifying issues that require escalation.

• Offer nursing support through member messaging related to symptoms, medications, side effects, chronic disease management, and established care plans.

• Collect relevant clinical data and provide appropriate education and guidance to members.

• Deliver evidence-based education on medication support, side effects, lifestyle recommendations, and self-management of chronic diseases.

• Recognize and communicate acute clinical findings, medication-related concerns, changes in member status, and other matters that necessitate provider or care team attention.

• Coordinate follow-up information to ensure continuity of care for members.

• Guarantee accurate and timely documentation of encounters in the designated electronic health record.

• Provide nursing services in accordance with applicable laws, regulations, professional standards, scope-of-practice requirements, and clinical and escalation protocols.

• Independently assess availability and accept service opportunities per the independent contractor agreement.

• Complete accepted visits or service commitments following clinical, legal, regulatory, documentation, privacy, and member-care standards.

⛳️ Requirements

• Active, unrestricted Registered Nurse license in good standing.

• Nurse Licensure Compact (NLC) multistate license or privilege required, as applicable.

• Ability and willingness to acquire and maintain additional state licenses necessary for contracted services, including licensure in relevant non-compact states.

• Bachelor of Science in Nursing (BSN) is mandatory.

• A minimum of five years of clinical nursing experience is required.

• At least one year of experience in providing care in a telehealth, virtual care, or ambulatory care setting.

• Two or more years of experience supporting individuals with diabetes, obesity, hypertension, dyslipidemia, or other cardiometabolic conditions is highly preferred.

• Certification as a Diabetes Care and Education Specialist (CDCES) or another relevant clinical certification is preferred.

• Strong clinical assessment skills, critical thinking abilities, and judgment for escalation.

• Capability to provide clear, evidence-based education to members within the RN scope of practice.

• Proficiency in electronic health records and technology that supports virtual member care.

• Excellent verbal and written communication skills, with the ability to simplify complex medical information into practical guidance for members.

• Ability to independently manage assigned visits, required clinical documentation, and related follow-up responsibilities.

• Contractors are responsible for maintaining all licenses, credentials, certifications, equipment, insurance, and other professional requirements relevant to the services they deliver.

• Authorization to work in the United States without visa sponsorship is required on the application form.
