This is a fully remote position, open to applicants in Tennessee.
• Manage, mentor, and provide support to a geographically distributed team of Care Coordinators catering to CHOICES members across Middle Tennessee.
• Supervise daily operations and offer clinical and operational direction.
• Assess staff performance, quality metrics, documentation standards, utilization benchmarks, and adherence to regulatory requirements.
• Ensure that members receive the necessary support and services to safely remain in their homes or nursing facilities.
• Offer hands-on assistance for complex, high-profile, and escalated member cases.
• Work collaboratively with providers, nursing facilities, community partners, and internal departments to meet member needs and fill service gaps.
• Coach and develop team members through training, case consultations, and performance management.
• Support workforce planning, onboarding, workload distribution, and initiatives for operational improvement.
• Accurately complete reports, audits, and deliverables in a timely manner.
• Provide direct assistance during member crises, challenging family interactions, and critical care coordination scenarios.
• Supervise, direct, and assess a diverse group of healthcare professionals.
• Communicate with and mentor care coordination team members to ensure adherence to policies, procedures, and guidelines for long-term and home- and community-based care.
• Occasionally manage a caseload of members as needed due to census changes or staffing coverage.
• Travel throughout the Middle Tennessee area as required by business needs.
• Minimum of 3 years of supervisory experience is required.
• At least 2 years of experience in the health insurance or managed care sector is necessary.
• Capability to perform root cause analysis, plan, organize, and coordinate multiple projects effectively.
• Proficient with Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint).
• Outstanding oral and written communication abilities.
• Strong interpersonal and organizational skills are essential.
• A valid driver's license is required.
• Registered nurse with a valid license in Tennessee or an active license in the applicant's state of residence if participating in the Nurse Licensure Compact Act; or a Master's in Social Work with an active unrestricted license.
• Willingness to travel 90% of the time during the day.
• Ability to manage direct member responsibilities while maintaining leadership oversight.
• Capacity to juggle multiple priorities and adapt to changing business demands.
• Employee participation in the Runzhimer Program (auto reimbursement plan) may be required.
• Various immunizations and/or associated medical tests may be mandated for this position.
