Case Manager, RN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversees the comprehensive interdisciplinary care plan for patients from the moment of admission through to discharge.

• Monitors patient care and serves as a point of contact among patients/families, healthcare staff, and insurance providers.

• Assesses patient requirements and available resources, recommending and facilitating optimal care plans for the best outcomes.

• Creates discharge plans utilizing available resources while promoting adherence to medical recommendations.

• Identifies patient care challenges and proposes modifications or new clinical pathways to enhance care quality.

• Recognizes patient risk factors, barriers to care, and potential discharge/readmission threats.

• Reviews care plans through chart analysis and patient interviews.

• Works in collaboration with the medical team to ensure smooth patient transitions within the healthcare system.

• Provides education to patients and their families regarding the care progression.

• Advocates for quality care and medically appropriate services.

• Manages ongoing insurance authorization processes for extended stays.

• Coordinates regulatory notifications and secures patient signatures as per policy requirements.

• Keeps up-to-date with insurance reimbursement regulations.

• Plans and implements discharge and transition strategies that address patients’ physical, social, and emotional needs.

• Complies with University and unit-level policies and procedures while ensuring the protection of University assets.

⛳️ Requirements

• Bachelor’s Degree

• Registered Nurse License (RN)

• At least 2 years of pertinent experience

• Proficient in both oral and written communication

• Capable of identifying, analyzing, and resolving various problems

• Skilled at analyzing, organizing, and prioritizing tasks under pressure while meeting deadlines

• Able to maintain effective interpersonal relationships
