This is a fully remote position, open to applicants in Pennsylvania.
• Engage in a collaborative process that includes assessment, planning, facilitation, care coordination, evaluation, and advocacy.
• Address the comprehensive health needs of individuals and families through effective communication and accessible resources.
• Enhance quality and cost-effective outcomes for Medicare beneficiaries.
• Conduct in-person case management activities within a specified geographic region as directed by leadership or in response to business requirements.
• Availability to work Monday through Friday, from 8:00 AM to 5:00 PM Eastern.
• Possession of an active and unrestricted Compact license.
• Preferred clinical practical experience of at least 3-5 years, including areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care with Medicare members.
• At least 2-3 years of experience in case management, discharge planning, and/or home health care coordination.
• Current Registered Nurse with an active state license in good standing in the region where job responsibilities are carried out.
• Bachelor’s degree or equivalent experience is required; must obtain a bachelor’s degree within 3-5 years as part of professional development.
• Willingness to travel occasionally within a designated area for in-person case management tasks.
• Bilingual candidates are preferred.
• Strong analytical and problem-solving abilities.
• Excellent communication, organizational, and interpersonal skills.
• Capability to work independently, including remote work arrangements.
• Proficient in MS Word, Excel, Outlook, PowerPoint, Microsoft Office 365, Teams, or similar Google Workspace applications.
• Competent computer skills, including the ability to navigate multiple systems and proficient keyboarding.
• Willingness to obtain multistate RN licenses if necessary; the company will provide support.
• Direct/hardwired internet connection to a modem/router within 7 feet of the computer.
• Minimum internet speed of 25 Mbps download and 3 Mbps upload.
• A quiet, secure, and private designated home workspace that complies with CVS Health and HIPAA guidelines.
• Ability to set up and utilize multiple monitors and manage multiple applications simultaneously.
• Capable of communicating via email, calendar invites, Teams messaging, and virtual meetings.
• Proficient in logging into secure systems such as VPN and EHR portals, managing lock screens, strong passwords, and identifying suspicious emails or links.
• Skillful in resolving common technical issues independently.
