Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Texas.

📋 Description

• Evaluate member requirements and create tailored care plans

• Coordinate care and connect members with suitable resources

• Recognize risks and eliminate obstacles to improved health

• Collaborate with multidisciplinary teams to achieve positive outcomes

• Assist members via telephone and occasional virtual interactions

• Enhance health outcomes for Costco members through case management

• Serve members across the nation as part of CVS Health’s healthcare services

⛳️ Requirements

• Active, unrestricted multi-state RN license in the state of residence

• Over 3 years of clinical RN experience

• Proficiency in using multiple systems, including Microsoft Office and other tools

• Availability to work Monday–Friday, 8 AM–5 PM CST, with occasional shifts from 10 AM–7 PM

• Willingness to obtain additional state licenses, with costs covered by the employer

• Associate degree in nursing

• Preference for candidates residing within 45 minutes of San Antonio, TX

• Integrated case management experience is preferred

• Bilingual candidates in Spanish/English are preferred

• BSN is preferred
