This is a fully remote position, open to applicants in Arizona, +9 more states.
• Act as the main coordinator for clients, collaborating with healthcare providers, specialists, pharmacists, social service agencies, and other key stakeholders.
• Manage client cases while coordinating healthcare benefits.
• Educate clients and ensure timely, cost-effective access to care.
• Work alongside caregivers, family support persons, providers, and Care Team members to foster wellness, recovery, independence, resilience, and member empowerment.
• Evaluate member needs across various areas including physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services.
• Supervise the creation of client care plans and goal-setting processes.
• Provide services in office settings, via telehealth, or in the field where members are most accessible.
• Connect clients with essential social services and support systems.
• Advocate for clients with healthcare professionals on their behalf.
• Implement evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and engagement initiatives, documenting activities in the CRM system.
• Assess client progress and revise SMART goals as necessary.
• Promote mental health awareness.
• Arrange transportation for clients.
• Ensure documentation and outcome measures are completed within care-plan deadlines.
• Keep patient health records up-to-date in the EMR and other business systems.
• Prepare monthly reports to ensure program compliance.
• Attend required training sessions.
• Report to the Director of Enhanced Care Management, ECM Clinical Manager, and/or ECM Program Manager.
• Minimum of 2 years of experience as a care manager, care navigator, or community health worker assisting vulnerable populations.
• Availability to work Monday-Friday, from 8:30 am to 5:00 pm Pacific Time, with flexibility for potential evening and weekend hours.
• Knowledge of government and community resources relating to social determinants of health.
• Clean driving record.
• Valid driver's license.
• Reliable transportation.
• Strong oral and written communication abilities.
• Positive interpersonal skills.
• Basic computer proficiency.
• Familiarity with Google Workspace, MS Office, and internet usage.
• Bilingual proficiency in English and Spanish is preferred.
• Applicants must reside in Arizona, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington.
• Must be legally authorized to work in the United States.
• Background check, which may include drug testing or other health screenings, is required prior to employment.
• Influenza vaccination is mandatory for roles involving patient, client, or customer interaction, with consideration for religious or medical exemptions.
