Case Manager

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Support patients in making well-informed choices regarding their clinical status, medications, and treatment options.

• Perform concurrent and retrospective assessments to evaluate medical necessity, service appropriateness, and care levels.

• Conduct interviews with clients or patients to assess their situational needs.

• Identify required services and support for patients.

• Record case history and compile reports.

• Act as a liaison between families and essential services.

• Apply care protocols, standards, and guidelines to guide decision-making.

• Coordinate with patients, physicians, insurance companies, and facility staff regarding clinical-related information and clarifications through phone, email, and other communication methods.

• Ensure that case reports remain accurate and current.

• Recognize and appropriately report any quality of care concerns.

• Perform additional ad-hoc responsibilities as assigned.

⛳️ Requirements

• Must hold a valid USRN - NY nursing license.

• A graduate degree in BS Nursing or a related field is required.

• A minimum of 2 years of clinical experience is preferred.

• Exceptional English communication skills, both written and verbal.

• Experience working with individuals who may struggle to express their needs.

• Outgoing personality and a team-oriented approach.

• Ability to thrive in a fast-paced environment.

• Strong critical thinking skills with a focus on customer service.

• Degree in Nursing is mandatory.

• Access to a reliable computer and noise-canceling headset.

• Second monitor for improved productivity.

• A stable internet connection is essential.

• Backup internet connection is required.

• Availability from Monday to Friday, 9 am to 6 pm US Eastern Standard Time (EST).
