Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver extensive case management services to elderly individuals and adults facing intellectual, developmental, or physical disabilities.

• Evaluate the health and safety requirements, strengths, interests, and preferences of members to create detailed Member Care Plans.

• Collaborate with a Field Care Nurse to meet members' health and safety needs in the least restrictive setting.

• Perform in-person social assessments upon enrollment and at least biannually, typically at the residences of members.

• Conduct quarterly face-to-face visits and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure the delivery of cost-effective services.

• Assess risk factors and provide education to members.

• Keep precise case notes, service authorizations, and updates to Member Care Plans.

• Travel to client locations or assignments and occasionally to Humana offices for training or meetings.

⛳️ Requirements

• Bachelor's degree in human services or a related discipline, along with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in a different field combined with at least 3 years of relevant experience.

• Possession of a valid state driver's license.

• Proof of personal vehicle liability insurance with a minimum coverage of 100/300/100.

• Preference for candidates residing within 45 minutes or less of the designated coverage area.

• Experience in Case Management is preferred.

• Familiarity with electronic case note documentation is preferred.

• Knowledge of community health and social service agencies, as well as additional community resources, is preferred.

• Willingness to travel up to 40% of the time.

• Availability to work Monday through Friday, from 8:00 am to 4:30 pm CST.

• Minimum home internet speed of 25 Mbps download and 10 Mbps upload is required for home or hybrid home/office employees.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information is necessary.

• TB screening is mandatory for this patient-facing position.
