Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver comprehensive case management services to elderly individuals and adults with intellectual, developmental, or physical disabilities within the Wisconsin Family Care program.

• Assist members residing in Dane/Columbia and the neighboring areas.

• Evaluate members' health and safety requirements, strengths, interests, and preferences to create a detailed Member Care Plan (MCP).

• Collaborate with a Field Care Nurse to meet members' health and safety needs, ensuring support in the least restrictive environment.

• Perform face-to-face social assessments at the time of enrollment and at least every six months, usually conducted at members' homes.

• Execute quarterly in-person visits and maintain monthly telephone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Guarantee cost-effective service delivery.

• Assess risk factors and provide education to members.

• Keep precise case notes, service authorizations, and updates to the MCP.

• Travel up to 40% within Dane and Columbia counties and adjacent regions.

• Report directly to the Manager of Care Coaching.

⛳️ Requirements

• Bachelor’s degree in human services or a related discipline, with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in a different field combined with at least 3 years of relevant experience.

• A valid state driver's license is required.

• Proof of personal vehicle liability insurance with minimum coverage limits of 100/300/100.

• Preferred: Living within 45 minutes or less of the designated coverage area.

• Preferred: Experience in case management.

• Preferred: Familiarity with electronic case note documentation.

• Preferred: Understanding of community health and social service agencies, along with additional community resources.

• This is a patient-facing position that requires TB screening for selected candidates.

• A dedicated workspace free from ongoing interruptions is necessary to safeguard member PHI / HIPAA information.

• Candidates should have self-provided internet service with a minimum of 25 Mbps download and 10 Mbps upload speeds.
