Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to determine their strengths, interests, and preferences.

• Create detailed Member Care Plans (MCPs).

• Collaborate with a Field Care Nurse to meet the health and safety needs of members.

• Perform in-person social assessments during enrollment and at least once every six months, usually at members' homes.

• Conduct quarterly face-to-face visits and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical needs.

• Ensure the delivery of services is cost-effective.

• Assess risk factors and provide educational resources to members.

• Keep precise case notes, service authorizations, and updates on MCPs.

• Deliver case management services to frail elders and adults with intellectual, developmental, or physical disabilities within the Wisconsin Family Care program.

• Occasionally travel to Humana offices for training or meetings.

⛳️ Requirements

• A Bachelor's degree in human services or a related field, along with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in another discipline with at least 3 years of relevant experience.

• A valid state driver's license.

• Proof of personal vehicle liability insurance with limits of at least 100/300/100.

• Preference for candidates residing within 45 minutes or less of the designated coverage area.

• Case Management experience is preferred.

• Familiarity with electronic case note documentation is preferred.

• Knowledge of community health, social service agencies, and additional community resources is preferred.

• Availability required Monday through Friday, from 8:00 am to 4:30 pm CST.

• Willingness to travel up to 40%.

• Capability to pass TB screening.
