Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to determine their strengths, interests, and preferences, and create a thorough Member Care Plan.

• Collaborate with a Field Care Nurse to deliver services that address the health and safety needs of members.

• Offer assistance in the least restrictive setting as per the Member Care Plan.

• Perform in-person social assessments at enrollment and at least every six months, usually at the members' homes.

• Conduct quarterly face-to-face visits and maintain monthly phone communication with members.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical requirements.

• Ensure efficient and cost-effective service delivery.

• Assess risk factors and provide education to members.

• Keep precise case notes, manage service authorizations, and update the Member Care Plan as needed.

• Serve members located across Green Lake, Marquette, Adams, and Waushara Counties.

• Travel up to 40%.

⛳️ Requirements

• A Bachelor's degree in human services or a related discipline, along with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in a different field accompanied by at least 3 years of relevant experience.

• A valid state driver's license.

• Proof of personal vehicle liability insurance with at least 100/300/100 coverage limits.

• Preferred residency within 45 minutes or less of the designated coverage area.

• Experience in Case Management is preferred.

• Familiarity with electronic case note documentation is preferred.

• Knowledge of community health and social service agencies, as well as additional community resources, is preferred.

• Capability to meet home internet requirements: a minimum of 25 Mbps download and 10 Mbps upload.

• Ability to work from a dedicated space that minimizes interruptions to safeguard member PHI / HIPAA information.

• Required TB screening for this patient-facing position.

• Availability to work Monday–Friday, from 8:00 am to 4:30 pm CST.
