Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Assist members within the Wisconsin Family Care program as part of the Inclusa/Humana team.

• Deliver comprehensive case management services to elderly individuals and adults with intellectual, developmental, or physical disabilities.

• Evaluate members' health and safety requirements, strengths, interests, and preferences to create Member Care Plans.

• Collaborate with a Field Care Nurse to meet members' health and safety needs in the least restrictive environment.

• Perform in-person social assessments upon enrollment and at least every six months, typically at members' homes.

• Conduct quarterly face-to-face visits and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure effective and economical service delivery.

• Assess risk factors and provide educational resources to members.

• Keep detailed case notes, service authorizations, and updates to Member Care Plans.

• Travel primarily within Juneau County and its surrounding areas.

⛳️ Requirements

• A Bachelor's degree in human services or a related discipline, along with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in a different field accompanied by at least 3 years of relevant experience.

• A valid state driver's license is required.

• Proof of personal vehicle liability insurance with coverage limits of at least 100/300/100.

• Preference for candidates residing within 45 minutes of the designated coverage area.

• Experience in Case Management is preferred.

• Familiarity with electronic case note documentation is also preferred.

• Knowledge of community health and social service agencies, along with other community resources, is preferred.

• Availability to work Monday through Friday, from 8:00 am to 4:30 pm CST.

• Willingness to travel up to 40% within primarily Juneau County and surrounding areas.
