Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Assess the needs and requirements of members to promote or sustain optimal wellness.

• Direct members and their families to suitable care and wellbeing resources.

• Organize services and resources to aid members in achieving their optimal wellness.

• Detect and address obstacles that impede effective care.

• Supervise patient care through evaluations, data analysis, member discussions, and proactive care planning.

• Ensure patients are progressing toward their desired health outcomes.

• Make decisions about work strategies and priorities while adhering to guidance and established protocols.

• Safeguard member PHI/HIPAA information while operating from a designated workspace.

• Occasionally travel to Humana offices for training sessions or meetings.

⛳️ Requirements

• Bachelor's degree in a health and human services discipline.

• A minimum of 2 years of relevant experience.

• Proficiency in Microsoft Office Word and Excel.

• Outstanding verbal and written communication and interpersonal abilities.

• Capability to utilize various electronic information applications/software programs.

• Required tuberculosis (TB) screening for this role that involves patient interaction.

• Valid driver's license for the state.

• Personal vehicle liability insurance that meets the specified minimum limits or state requirements.

• A separate room with a locked door designated for use as a home office.

• Previous experience with Medicare & Medicaid beneficiaries is preferred.

• Familiarity with electronic case note documentation and multiple computer applications/systems is preferred.

• Experience in health promotion, coaching, and wellness is preferred.

• Understanding of community health and social service agencies as well as community resources is preferred.

• Home internet connection with a minimum of 25 Mbps download and 10 Mbps upload speed.
